Summary of the project

Due to their complex problems and the resulting difficulty of adequate and, above all, seamless further treatment planning in the inpatient context of psychiatry, people who are addicted to addictive substances carry considerable weight. Their often precarious life situations require early interdisciplinary cooperation and coordination. The project is embedded as a master's thesis in a larger study by the Department of General Psychiatry at Rheinhessen Fachklinik Alzey. It aims to analyse the internal interfaces from admission to discharge and thus to professionalise the discharge management of patients with addiction. With the help of a set of diagnostic tools designed on the basis of the ICF, the particularly problematic, i.e. aftercare-intensive clientele is to be identified in order to be able to refer them directly to discharge management, which is to be standardised as part of this project. The results of these processes, which are then written down in a concept, are to be tested, documented and finally evaluated on approx. 8-10 patients.
